RECOMMENDED RESOLUTION: An air gap between the water supply inlet and the flood level rim of the plumbing fixture, equipment, or nonfood equipment shall be at least twice the diameter of the water supply inlet and may not be less than 25 mm (1 inch).
OBSERVATIONS & CORRECTIVE ACTIONS: At the time of the re-inspection the air gap was not provided at the bar floor drain for the sinks, and ice bin.

RECOMMENDED RESOLUTION: Bait stations have been installed. Remove grease and food from under the cook line equipment. Monitor bait stations and clean up droppings.
OBSERVATIONS & CORRECTIVE ACTIONS: At the item of the re-inspection the establishment has mouse droppings under the counter, under the equipment in the service area, in the shelving along the stairs to basement, and in the basement behind food shelves and refrigerators.
