Employee made a new bucket and it was tested to ensure that it was at the correct concentration. Observed that there was no sanitizing bucket setup on the prep line in the kitchen. OBSERVATIONS & CORRECTIVE ACTIONS: Observed excessive mold build up on a card board box that was holding canned foods on one of the storage racks in front of the Walk-in Cooler downstairs. Observed grease and food build up on the table that holds the Flat Grill in the kitchen. Observed some food build up in the containers on the shelf below the waffle iron and warmers by the Flat Grill. Observed excessive dust build up on the fan guards on the condenser in the True reach-in Cooler upstairs. Observed excessive build up in the door seals of the True Reach-in Cooler upstairs. Observed excessive build up on the storage racks to the left of the True Reach-in Cooler upstairs. Observed food build up on the shelf that holds the microwave by the Flat Grill. Observed dried fod build up on some of the speed racks in the kitchen. Observed build up inside of the containers that hold the napkins and wrapped utensils on the speed rack by the 3 compartment sink. Employee washed out the build up. Observed excessive white build up on the numerous storage shelves downstairs in the back storage areas. Observed some build up on the low shelf of the Prep Table downstairs. Employee stored the egg wash correctly in a manner not to cause cross contamination. Observed an employee crack open shelled eggs with his gloves on and then did not remove gloves and wash hands before touching other items. Discussed with the employee and he removed the gloves, washed hands and rewashed the contaminated items. Observed an employee place containers of food in the sink downstairs which has drain lines going into the sink by the hand washing sink downstairs. Employee removed the foods from the sink.

Observed excessive build up on the floor underneath the True Reach-in Cooler. Observed some cracked floor tiles by the True Reach-in Cooler, by the 3 compertment sink and also in various other areas. Observed dried food build on the wall to the left of the True Reach-in Cooler. Observed that the low walls underneath the dishwashing area and also by the mop sink that are cracked and chipping. Observed excessive build up on the low walls underneath the dishwashing area. Observed excessive debris on the floor underneath the dishwashing area. Observed that some of the walls underneath the 3 compartment sink are in disrepair. Observed some dust build up on the ceiling vent in front of the mop sink upstairs. Observed excessive mold build up on the walls above and below the 3 compartment sink. Observed dust build up on the wall above the dishwasher. Observed that the paint on the wall by the back exit door is chipping. Observed excessive build up on the walls in the bakery prep area upstairs. Observed excessive debris build up on the floors throughout the offices downstairs. Observed food and debris build up on the floor underneath the storage racks in the Walk-in Freezer. Observed excessive dust and debris build up on the floors underneath the storage racks downstairs in the back storage area. Observed excessive debris build up on the floors surrounding the grease trap downstairs. Observed some build up on the top of the grease trap. Observed excessive dust build up on the fan guards on the condensers in the Walk-in Cooler. Observed some debris build up on the floors underneath the racks in the Walk-in Cooler.
