Observed that the low walls underneath the dishwashing area and also by the mop sink that are cracked and chipping. Observed excessive build up on the low walls underneath the dishwashing area. Observed some dust build up on the ceiling vent in front of the mop sink upstairs. Observed excessive mold build up on the walls above and below the 3 compartment sink. Observed dust build up on the wall above the dishwasher. Observed excessive build up on the walls in the bakery prep area upstairs. Observed excessive debris build up on the floors throughout the offices downstairs. Observed food and debris build up on the floor underneath the storage racks in the Walk-in Freezer. Observed excessive dust and debris build up on the floors underneath the storage racks downstairs in the back storage area. Observed excessive debris build up on the floors surrounding the grease trap downstairs. Observed some build up on the top of the grease trap. Observed excessive dust build up on the fan guards on the condensers in the Walk-in Cooler. Observed some debris build up on the floors underneath the racks in the Walk-in Cooler. Observed some debris build up on the floor underneath the ice machine upstairs. Observed some debris build up on the floor to the right of the Front Reach-in Cooler by the cash register. Observed some dried food build up on the walls throughout the entire upstairs and downstairs. Observed debris build up on the floors throughout the entire upstairs and downstairs.

Observed excessive dust build up on the fan guards on the condenser in the True reach-in Cooler upstairs. Observed excessive build up on the storage racks to the left of the True Reach-in Cooler upstairs. Observed excessive white build up on the numerous storage shelves downstairs in the back storage areas. Observed some build up on the low shelf of the Prep Table downstairs. Observed numerous hairs and build up on the storage racks downstairs by the employee restroom. Observed some dust build up on the fan guard on the condenser of the Front Reach-in Cooler by the cash register. Observed some build up on the storage racks in the Walk-in Cooler downstairs. Observed excessive dust build up on the blowing fan in the window by the Flat Grill.
5-205.15	Plumbing system not maintained in good repair * CRITICAL Comply by : Apr 05, 2015 RECOMMENDED RESOLUTION: OBSERVATIONS & CORRECTIVE ACTIONS: Observed a large leak coming from the backflow device at the mop sink upstairs. Observed a leak coming from the plumbing in the ceiling to the left of the hand washing sink downstairs.

CRITICAL Corrected On Site RECOMMENDED RESOLUTION: OBSERVATIONS & CORRECTIVE ACTIONS: Observed sliced tomatoes sitting out at room temperature on the prep table upstairs. Discussed with the employee and he discarded the tomatoes. Food is listed in the temperature section and notated with a double *.
