RECOMMENDED RESOLUTION:
OBSERVATIONS & CORRECTIVE ACTIONS: Observed the side door to kitchen entrance, and the front entrance door for customers, to be propped open at time of inspection. Doors do not have a screen door to protect from entry of pests and environmental contaminents.

Employee had dishes stored in sanitize basi, with only tap water. Employee added chlorine bleach solution to 3 compartment sink, at correct range of 50-100ppm.

OBSERVATIONS & CORRECTIVE ACTIONS: Observed several dead roaches of all sizes, and a dead mouse on glue trap, under the hand wash sink. Observed 3 live adult roaches crawing on the floor by and under fryer. Observed 3 live adult roaches crawing on the wall, under and behind the 3 compartment sink. Observed live baby roach on floor by side entrance door. Observed 1 live juvenille roach on drain lines, under the 3 compartment sink.

OBSERVATIONS & CORRECTIVE ACTIONS: Observed hamburger patty to be hot holding below 135F degrees inside steam well, next to pizza oven. Employee stated item hot holding under the 4 hour window, and reheated. Added more water to steam well, so water touches sides and bottom of the pan.
