Observed mold debris build pu on wall to right of ware wash machine. Observed mold debris build up on wall below and behind ware wash machine. Observed mold on wall to left of mop sink. Observed excessive grease build up on floor below and behind cooking equipment.

OBSERVATIONS & CORRECTIVE ACTIONS: Observed utensils being stored in containers with debris build up, on grain storage bins to left of cooking equipment. Utensils and bin removed for cleaning. Observed ware wash employee accidentally drop scoop after washing in three vat sink, into a pan of dirty water and then place into sanitizer basin. Scoop removed for re-washing.

RECOMMENDED RESOLUTION:
OBSERVATIONS & CORRECTIVE ACTIONS: Observed a gallon of milk stored on shelf below prep table was not maintaining temperatures of 41F or lower. Milk moved to cooler by manager. Observed tubs of raw chicken and raw fish and fry station were not maintaining temperatures of 41F or lower. Per employee, foods had only been out for 20 or so minutes and only a small amount of ice was added for cold holding. Fish and chicken placed in cooler for rapid cooling. NOTE: If inspector observed raw chicken and raw fish not holding proper temperatures at fry station ice tubs, establishment will no longer be allowed to hold foods without mechanical refrigeration. Observed raw shrimp in walk in cooler not holding temperatures of 41F or lower. Temperatures suggest shrimp had been out of temperature for an extended period of time. Shrimp discarded. Other foods in walk in cooler at 41F or lower.
