OBSERVATIONS & CORRECTIVE ACTIONS: Floors in kitchen are in disrepair, uneven, unsealed wood. No longer smooth, easily cleanable, durable or nonabsorbent. Kitchen floor is in disrepair by handsink and maketable area. Concrete flooring in walkin cooler is not smooth, easily cleanable, durable, contains chipping and dents. Holds standing water after mopped at 430p this evening. Floors in basement are in same condition as walkin cooler and are not smooth, easily cleanable. Observed cooking pot and chafing dishes in subbasement area with no solid flooring. Wooded slats are in place for flooring. Foundation material is the material for walls and ceiling area. Back wall by mop sink is in disrepair.

Observed large plastic gallon buckets in kitchen are burned and no longer smooth, easily cleanable. Observed several bent and uneven areas of fryer baskets at fryer. Rubber spatulas in red rack by dish machine contain chipping, burn marks and is no longer smooth, easily cleanable.
