OBSERVATIONS & CORRECTIVE ACTIONS: Observed that the floor tiles in the hallway by the restroom is badly cracked and damaged. Observed some dried food build up on some of the walls in the kitchen. Observed excessive food and debris build up behind equipment on the floors in the kitchen. Observed dirt build up on the floor in the restroom. Observed debris build up on the floor in the mop sink room. Observed excessive dirt build up in the mop sink. Observed some water stained ceiling tiles in the kitchen. Observed holes in the door leading to the kitchen. Observed numerous gaps in between numerous pieces of equipment like cabinets and counter tops and walls throughout the entire facility.

Observed a dead roach on the wall underneath the small prep shelf by the entrance to the cash register area. Observed numerous roaches in the cabinet and hinges of the cabinet next to the Make Table Cooler. Observed roach feces on the cabinet by the Make Table Cooler.
