OBSERVATIONS & CORRECTIVE ACTIONS: Observed some sticky build up on the floor in the Meat Walk-in Cooler. Observed a very strong smell of old meat in the Meat Walk-in Cooler. Observed some debris build up on the floor by the CO2 tank downstairs. Observed some food build up on the floor underneath the storage racks in the Walk-in Freezer. Observed excessive dried food build up on the wall by the fire exstinguisher on behind the Coke Reach-in Cooler next to the Reach-in Freezer. Observed some dust build up on the ceiling in the dishwashing area. Observed some black build up on the wal below the back hand washing sink in the kitchen. Observed some dried food build up on the wall to the right of the small oven on the prep table by the mop sink. Observed some grease build up on the wall behind the cooking equipment underneath the venthood. Observed that the paint on the ceilings downstairs are chipping in various areas and also in the liquor store room downstairs. Observed numerous water damaged and stained ceiling tiles downstairs. Observed excessive dust build up on and around the ceiling vents on the front prep line. Observed some dried food build up on the ceiling and wall by the serving window on the front prep line. Employee cleaned off the build up. Observed excessive build up on the floor underneath the Make Table Cooler on the front prep line. Observed excessive build up on the floor underneath all of the cooking equipment on the front prep line.

OBSERVATIONS & CORRECTIVE ACTIONS: Observed excessive dried food build up on the underside of the shelf above the steam wells on the front prep line. Observed excessive food build up in the containers that hold the clean utensils on the low shelf of the prep table to the left of the dishwasher.
