OBSERVATIONS & CORRECTIVE ACTIONS: Reach-in cooler by the drive thru area is holding temperatures above 41 degrees. Person in charge is aware of the issue and they already had condiments on ice in the cooler.

OBSERVATIONS & CORRECTIVE ACTIONS: Observed heavy dust buildup on the ceiling vents above food prep area. Observed black buildup on ceiling vent above dry storage area. Observed broken/coving and caulking in/around walk-in cooler. Observed buildup at floor to wall juncture to the left of the cookline. Observed caulking above coving in disrepair to the left of cookline. Observed buildup on the wall and floor under three compartment sink. Observed grease buildup on the floor under the fryer. Observed debris under the salsa bar. Observed black buildup on and around the women's restroom ceiling vent. Observed water stained ceiling in the women's and men's restrooms.

OBSERVATIONS & CORRECTIVE ACTIONS: Observed rusty shelving in the drive thru area reach-in cooler, small make table cooler, walk-in cooler, and dry storage shelving.

At time of inspection by authorized REGULATORY AUTHORITY inspectors: (1) A certified food service operations manager must be present in the food service area and on duty at all times of business operation, and must present a FOOD MANAGER PERMIT or card to the authorized REGULATORY AUTHORITY inspector at the time of inspection, or; (2) For food service operations where no manager is certified, eighty percent (80%) of required food handler staff shall obtain a (3) three years FOOD HANLDER CARD from the City of Kansas City, Missouri Health Department either upon completion of a FOOD HANDLER'S course through the Health Department or upon the Health Department's approval of a comparable FOOD HANDLER PERMIT or card from another provider.
