OBSERVATIONS & CORRECTIVE ACTIONS: Observed bowls used as scoops in the container of sushi rice. Observed ice scoops stored with the handle touching the ice at the ice basins in the server station. Ice scoops were moved to have the handle upwards. Observed the rice paddle at the buffet line rice warmer to be sitting in standing water. Water was poured out. Observed the ice cream scoop stored in standing water and the dipper well not turned on. Observed bowls used as scoops and handles touching food product in the bulk food containers by the can rack.

Observed the light fixture in the Pepsi cooler, near the make table, to be falling from the top of the cooler. OBSERVATIONS & CORRECTIVE ACTIONS: Observed dust buildup on the ceiling, walls, and light fixtures throughout the dining room. Observed stains on the ceiling tiles in the utility room, dining room, and throughout kitchen. Observed dust buildup on the ceiling vents in the dining room, especially over the buffet area. Observed heavy debris on the floor of the utility room. Observed green substance on the right side of the large middle window facing Mill Street. Observed the window frame is in disrepair. Observed dust buildup was observed on the exhaust vent on the ceiling above the stalls in both men's and women's restrooms. Observed buildup and debris on the floors of the men's and women's restroom. Observed splatter on the wall, to the right of the three-compartment sink. Observed plastic wrap on a GFCI outlet to the right of the three-compartment sink that has food residue on it. Observed the entry/exit doors to the kitchen to be in disrepair with holes in the doors and duct tape covering the doors. Observed buildup on the floor below the wok line. Observed buildup and debris in the drains on the floor at the wok line and below the front handsink. Observed buildup on the mirror, floor, and walls in the staff restroom. Observed dust on the exhaust vent on the ceiling of the staff restroom. Observed the paint on the door to the staff restroom to be peeling. Observed buildup and debris on the floor below the dish machine and soaking sinks. Observed buildup on the wall behind the dish machine. Observed buildup on the wall below the hibachi flat top and sushi station. Observed standing water on the floor with a green tint below the sushi station.

Observed cantaloupe on the salad bar with internal temperatures above 41F (see temp log). Food was removed to the walk-in cooler as manager stated it was recently cut before opening for service and within the 4 hour window. Observed sushi rolls on the sushi ice table to have internal temperatures above 41F (see temp log). Manager stated that they were recently made but could not confirm a time. Foods were discarded.

Chicken was removed to food-grade containers. Observed a broken plastic bucket being used to store jalapenos on the make table. The bucket was replaced with a metal container. Observed some duct tape on gray bus tubs stored in the make table Pepsi cooler.
