OBSERVATIONS & CORRECTIVE ACTIONS: Observed dust buildup on the ceiling, walls, and light fixtures throughout the dining room. Observed dust buildup on the ceiling vents in the dining room, especially over the buffet area. Observed green substance on the right side of the large middle window facing Mill Street. Observed the window frame is in disrepair. The green substance and window frame have been covered with duct tape. Observed dust buildup was observed on the exhaust vent on the ceiling above the stalls in both men's and women's restrooms. Observed dust on the exhaust vent on the ceiling of the staff restroom.
