OBSERVATIONS & CORRECTIVE ACTIONS: Observed au jus (112°F) in the serve line soup well to be at unsafe temperature. Au jus had been there less than two hours so was taken to be reheated and replaced into the soup well after the heat setting had been adjusted for proper holding. **See Temperature Observations.

OBSERVATIONS & CORRECTIVE ACTIONS: Observed cook line employee plating chicken fried steak, with the steak and the gravy coming into contact with the employee's bare hands (particularly the thumb) while being plated. Observed food employee using bare hands to plate cucumbers onto salads for service to consumers. Employee was informed of this issue and cucumbers were removed and replaced with new cucumber slices handles properly with single-service gloves as a barrier.

Observed extreme frost buildup on the walk-in freezer door inside the chicken walk-in cooler, such that the freezer door no longer closes. Observed extreme frost buildup inside the walk-in freezer, on the floor, around the door, on foods, on shelves, etc.
