OBSERVATIONS & CORRECTIVE ACTIONS: Observed cod inside the make table reach in cooler, to be cold holding above 41F degrees. Person in charge stated item was removed from walk in cooler and stocked in make table under the hour, was out for approximately 20 minutes during stocking. Under the 4 hour cold holding window, item to remain in reach in cooler.

OBSERVATIONS & CORRECTIVE ACTIONS: Observed cracked floor tiles by men's restroom entrance. Observed broken floor tiles through out the server area, and under the soda machine. Person in charge stated a bid has been placed to redo kitchen/server area flooring and should be completed prior to year's end. Observed grease build up on the wall, behind the grill.
