OBSERVATIONS & CORRECTIVE ACTIONS: Observed tomatoes, pico de gallo, cheese, mayo and ambient temperatuer of make table/cooler were not maintaining temperatures of 41F or lower. Per manager, he noticed that the make table/cooler was not working properly and called in a work order this morning. Inspector observed multiple foods in kitchen walk in cooler were not maintaining temperatures of 41F or lower. Per manager, foods observed out of temperature were foods in make table/cooler that was not working properly, moved to walk in cooler. Foods observed out of temperature discarded. Inspector observed all other foods were at 41F or lower. Inspector observed salas at salsa bar ice bath were not maintaining temperatures of 41F or lower. Inspector that salsa containers were sitting on a bed of ice and water, to where just bottom of salsa containers were in contact with ice. Ice and water added to food level of salsa containers.

OBSERVATIONS & CORRECTIVE ACTIONS: Observed several containers of rice that were, per manager, just done cooking around 30 minutes prior, were not cooled to the proper temperature to be placed in cooler. Temperatures suggest rice had not been out for an extended period of time after cooking. Rice placed in cooler by manager. Inspector observed two containers of chicken cooling on shelf below kitchen prep table, that had not been cooled properly and placed in cooler at appropriate temperature. Temperatures suggest chicken had not been out for an extended period of time. Chicken placed in cooler by manager.
