OBSERVATIONS & CORRECTIVE ACTIONS: 3 live cockroaches inside empty screw holes in drywall near the kitchen hand sink. 2 live cockroaches in and behind the laminate wall panel at kitchen hand soap dispenser. 1 live cockroach between FRP panel and drywall at warewash area. 4 cockroaches on the walls around meat saw and warewash area. 1 live cockroach between electrical conduit and wall near meat saw. 1 live cockroach on the Ansul switch housing near meat saw. 3 live cockroaches between the right side door frame and wall, 7 on between the left side door frame and wall on the kitchen side. 6 live cockroaches inside the kitchen door frame on the POS side. 1 live cockroach between the wall and Ansul conduit. Dead cockroach in the chest freezer. Very large adult cockroach on the floor at cook line. Live cockroach on the floor near the unused steam table. 5 live cockroaches in water heater closet. 1 live cockroach in trash cabinet. 2 dead cockroaches between the door and door jam of HVAC closet. 1 dead cockroach on floor of HVAC closet. Dead cockroaches and fragments inside condenser units of chest freezers. Heavy frass build-up on walls around FRP panel seems, Ansul switch, freezer gaskets, electrical outlets and covers, electrical conduits, cooler condenser areas. Mouse droppings on floor behind fryers, North wall of dining room, behind chest freezers, and in dry storage area. Heavy droppings and nesting materials on floor in front area behind unused Coke 1-door cooler. Mouse entry points in wood bases throughout.
