OBSERVATIONS & CORRECTIVE ACTIONS: Observed debris and residue build up on floor, wall and plumbing below three vat sink. Observed bottom corner tiled wall, below three vat sink is in disrepair. Observed floor tiles below right drain board of ware washing machine in disrepair. Observed wall, just below ceiling, at top of staircase in disrepair. Observed wall to right of staircase, ascending, is with debris and residue build up. Observed a large circular hole in floor of downstairs uniform room, near entrance way. Observed rectangular holes in pantry ceiling, above single service cups shelving unit. Observed exposed insulation in pantry, above chemicals and extra equipment storage shelves.

OBSERVATIONS & CORRECTIVE ACTIONS: Observed a large, deep container of pelotes cooling in walk in cooler, and not in shallow container and without being stirred to rapidly cool. Manager had employee stir pelotes every 5-10 minutes for rapid cooling.
