Discussed correct date marking procedure for frozen/thawed foods and manager corrected label to reflect accurate dating.

Observed buildup on the walls in the walk-in cooler. Observed damaged wall near light switch between basement stairs and storage room. Observed buildup on the wall behind equipment at the front service area. Observed multiple water stained/damaged ceiling tiles in the basement storage area. Observed buildup on the floor in the basement walk-in cooler under the shelving. Observed heavy buildup on the floor under the cookline equipment/pizza oven and on the walls behind them. Observed a hole in the wall behind the pizza make table. Observed caulking at the top of the three compartment sink in disrepair with black buildup. Observed black buildup on the FRP wall board behind the dishwash machine. Observed heavy buildup on the floors/walls and floor drain under the three compartment sink and dishwasher.

Manager stated that is does not work, is not plugged in at this time. They are planning to replace it. No food is stored in the cooler and the items at the top of the make table are stored on ice. Observed cutting boards at make tables with deep grooves in them. Observed wet towel in the bottom of the make table cooler.

OBSERVATIONS & CORRECTIVE ACTIONS: Observed dead mouse on the floor in the basement near the stairwell next to soda bottle storage. Manager removed mouse at inspection. Observed mouse droppings in the basement along walls, under storage racks, and under the stairwell. Observed mouse droppings along the walls of the dishware storage space and along walls in dining area. Pest control services were last at the establishment on 2/2/16. Pest control noted several gaps/holes to be filled. They are treating with traps and bait.
