OBSERVATIONS & CORRECTIVE ACTIONS: Observed a box of onion rings stored on the floor covered in ice in the walk-in freezer. Manager opened the box and revealed the bags of onion rings to be clear of ice. Manager moved onion rings to a shelf.

Observed cook touch cell phone and then put on gloves without washing hands first. Employee was instructed to wash hands anytime contamination occurs. Observed cook cough into elbow and resume food preparation without washing hands. Employee was instructed to wash hands again.
