OBSERVATIONS & CORRECTIVE ACTIONS: Observed some build up on the floor underneath the three compartment sink. Observed some build up on the walls behind the stove and Flat Grill. Observed some build up on the wall to the left of the stove. Observed excessive build up on the floors underneath all of the equipment throughout the kitchen. Observed standing water on the floors by the mop sink and also in the hallway leading to the restroom. Upon discussion with the owner he stated that the water is leaking from the window in the restroom when it rains. Owner must get the window fixed immediately. Observed a large puddle of standing water on the floor in the basement. Observed a strong mildew smell in the basement from leaking water. Observed some black build up on the wall in the restroom. Observed some debris build up on the floor in the restroom.

Observed some dried build up on the back of the handles of the Reach-in Freezer and Reach-in Cooler. Observed some build up on the fronts of the Reach-in Freezer and Reach-in Cooler. Observed a large ice sickle hanging from the drainage line on the condenser in the Reach-in freezer.
