OBSERVATIONS & CORRECTIVE ACTIONS: 4 live cockroaches observed coming from suspended ceiling tiles above the warewash sink. 4 live adult cockroaches observed around the consumption ice well, and crawling on the soda fountain drain grates. Live nymph cockroach on wall near dry storage. Live cockroach between wall and broken FRP at dry storage. 3 Live cockroaches observed around the dials of cooking equipment at cook line. Heavy frass build-up observed throughout warewash area, in ceiling corners around dry storage and prep areas, on shelving behind microwave. Live fruit flies observed in food service area. Live house flies observed in food service area. Mouse droppings on floor behind sliding glass 2-door cooler.
