OBSERVATIONS & CORRECTIVE ACTIONS: Observed damaged/stained ceiling tiles in the restroom hallway and women's restroom. (these tiles must be replaced with smooth, easily cleanable, non-absorbent material). Observed hole in the wall above the kitchen hand sink. Observed exposed insulation in back kitchen prep/storage area. Observed buildup on the floor and walls in the kitchen around the cooking and prep areas. Observed soda syrup spillage on the floor in the basement under the soda bag in box storage rack.
