Person in charge stated all aforementioned were prepped on 3/24 and added date markings at the time of inspection. Observed 5 dozen, 3oz to-go cups of salsa and sour cream in the salad low boy without date marking. OBSERVATIONS & CORRECTIVE ACTIONS: Observed missing floor tiles throughout the following areas: women and men's basement bathrooms, main bar, back bar, and front dining room. Observed ceiling tiles throughout the kitchen to be stained and in disrepair. Observed missing ceiling tile above the grill hood vents. Observed a hole in the right wall of the basement employee bathroom. Observed build-up and debris under the 3 compartment sink, all ice-wells, and the handwashing sink of the main bar.
