OBSERVATIONS & CORRECTIVE ACTIONS: Observed all phf items being held cold in the make table top to be out of temp and being held at temps between 49-52 degrees farenheit. Items discarded by person in charge and temp on cooler was adjusted. Bulk phf items being held cold in the tall pepsi reach in cooler were being held between 46 and 54 degrees farenheit. Person in charge discarded items and adjusted temp on cooler.

Observed several damaged ceiling tiles in food prep area. Observed wall above exit door to have food debris/build up on it. Observed tiles behind 3-compartment sink to be in disrepair. Observed lower shelf surfaces of kitchen to have food debris/build up.
