Employee washed and sanitized the surface after removing the dead roach. Observed roach egg sacks and roach feces on, in and behind the old soap dispenser on the wall by the hand washing sink. Employee removed the soap dispenser and wiped down the wall. Observed some roach feces on the wall by the band saw. Observed some roach feces on the inner door frame of the Men's restroom.

Observed excessive build up and food stains on the walls throughout the entire kitchen and behind all equipment and in other areas. Observed excessive food and debris build up on the floors underneath and behind all equipment throughout the kitchen and other areas. Observed that the wall paint is chipping by the door in the Women's restroom. Observed numerous water damaged and water stained ceiling tiles throughout the dining area and also in the kitchen. Observed debris on the floor behind equipment in the front coffee area. Observed food build up on the plumbing underneath the three compartment sink. Observed excessive build up in the mop sink and underneath the three compartment sink.
