OBSERVATIONS & CORRECTIVE ACTIONS: Observed build up on inside the compartment where the glass racks are located at the front serving line. ***REPEAT VIOLATION will be assessed in 3-10 business days.*** Observed build up on the outside doors and handles of the 4 door reach in cooler. Observed build up on the bottom shelf of the far left reach in cooler in the front serving area. Observed build up in the cabinet below the left side soda dispensing machine. Observed build up on the glass storage racks to the right of the soda dispensing on the right side of the front serving area. Observed build up on the hood vents above the cooking equipment on the cooks line. Observed ice build up on the left side of the condenser unit of the walk in freezer.

OBSERVATIONS & CORRECTIVE ACTIONS: Observed heavy food debris and build up under all of the cooking equipment, prep table tops, and on the floor and walls under the dish machine on the front serving line. ***REPEAT VIOLATION will be assessed in 3-10 business days.*** Observed build up on the floor of the walk in freezer and perimeter of the floor of the walk in cooler. Observed a damaged door frame/wall to the left of the back exit door. Observed build up in the floor drain below the 3 compartment sink. Observed debris/food/cigarette butts on the floor in the back/prep-3 comp sink area.
