OBSERVATIONS & CORRECTIVE ACTIONS: Observed sushi rolls on the sushi bar to have internal temperatures above 41F. Manager stated that the sushi rolls were prepared at 1130am (showed inspector time stamp from the rice). The sushi rolls were put in the walk-in freezer to cool down. Observed raw beef, raw chicken, and raw squid on the hibachi buffet to have internal temperatures above 41F. Person in charge stated the food had been out for 1 hour and was removed to the walk-in cooler to cool down.

Observed stained and damaged ceiling tiles in the utility closet. Observed the wall in disrepair on both sides of the mop sink in the utility closet. Observed buildup on the wall by the soda BIBs in the utility closet. Observed heavy debris and buildup on the floor and wall below the dish machine.
