OBSERVATIONS & CORRECTIVE ACTIONS: Observed residue build up on bottom pots and pans shelf. Observed residue build up on lower wall, left of three vat sink. Observed residue build up on ledge below kitchen hand sink. Observed residue build up on exterior of back kitchen cabinet doors. Observed grease residue build up on floor below and and around stove. Observed grease on wall to right of stove. Observed grease on wall behind stove. Observed two circular holes in kitchen floor, in front of walk in cooler door. Observed residue build up throughout walk in cooler walls. Observed walk in cooler ceiling and wall, in front and left of walk in cooler door are in slight disrepair. Observed debris and residue build up on walk in cooler floor below far right retail shelving unit. Obsrved restroom floor, along walls are curling up. Observed restroom door and wall to right of door with residue. Observed mold build up on left exterior of walk in cooler door jam. Observed dining room ceiling is in disrepair in several places.

OBSERVATIONS & CORRECTIVE ACTIONS: Observed raw eggs and cheese under no type of temperature in kitchen were not at 41F or lower. Per employee, eggs and cheese were out for around 30 or so minutes. Temperatures of foods suggest that they had not been out for a long, extended period of time. Foods moved to walk in cooler.
