OBSERVATIONS & CORRECTIVE ACTIONS: Observed stains and dried food build up on the outsides of the coolers and freezers in the kitchen. Observed excessive dust build up on the fan guards on the condenser in the 3 Door Reach-in Cooler. Observed some build up in the sliding door tracks of the three door cooler in the kitchen. Observed some dried food build up on the lower portion of the small pizza oven and also on the inside catch tray. Observed grease build up on the sides of the Flat grill and fryers. Observed grease build up between the fryers. Observed grease build up on the surfaces of the fryer underneath it. Observed some food build up in the door seals of the refrigerator. Observed excessive grease build up underneath the metal ledges of the fryers in the kitchen.

Observed debris and old food build up on the floor underneath the wood looking prep table. Observed a hole in the ceiling by the electrical box that comes down from the ceiling in the kitchen. Observed a hole in the ceiling above the vent hood above the Flat Grill. Observed food build up on and around the door entering the kitchen that has the potato slicer on it. Observed stains and build up on the walls behind the three compartment sink in the kitchen. Observed some food build up on the top of the grease trap. Observed some debris and old food on the floor behind the grease trap. Observed stains and build up on the wall behind and to the left of the stove. Observed some broken floor tiles by the back exit door. Observed some broken floor tiles at the bar. Observed a hole in the bottom of the door by the office. Observed some small holes in the bottom right wall by the back exit door. Observed tape covering a broken electrical outlet on the wall to the left of the stove. Observed a hole in the wall and that the wall is in disrepair above the hand drying provisions in the Men's restroom. Observed a large hole in the wall behind the large ice machine. Observed some dust build up on the wall to the right of the large ice machine. Observed some exposed electrical wires extending out of the wall above the deep freezers. Observed grease build up on the wall behind the fryers.

OBSERVATIONS & CORRECTIVE ACTIONS: Observed excessive black build up on the metal shield on the inside of the large ice machine at the bar. Employee discarded some of the ice, covered the remaining ice and cleaned off the build up. Observed excessive grease build up on the inside and outside of the popcorn popper in the dining area. Instructed the manager to put an out of order sign on the popper until it can be soaked and cleaned. Observed dried food build up on the back side of the meat slicer on the prep table in the kitchen. Manager cleaned off the build up.
