OBSERVATIONS & CORRECTIVE ACTIONS: Observed some debris build up on the floor behind the reach-in freezer and storae rack next to it. Observed food and dirt build up on the low walls throughout the dishwashing area. Observed some food build up on the low wall by the mop sink. Observed some food build up on the walls behind the large ice machine. Observed food and grease build up on the walls behind the cooking equipment in the kitchen. Observed grease and food build up on the pipes behind the cooking equipment in the kitchen. Observed build up on the walls and floors throughout the entire kitchen.

OBSERVATIONS & CORRECTIVE ACTIONS: Observed several dead roaches on the floor by the back exit door. Observed dead roaches in all of the light shields throughout the kitchen. Observed dead roaches on the top of the hot water heater. Observed a dead roach on the thermostat above the prep table. Observed a dead roach on the floor by the grease trap. Observed a few dead roaches on the wall behind the hot water heater. Observed a dead roach on the left side of the Reach-in Freezer. Observed some mice droppings on the floor in the dry storage area behind racks and the Reach-in Freezer.
