OBSERVATIONS & CORRECTIVE ACTIONS: Standing water along soda station flooring; trash can area. Food spillage throughout corners of Establishment. Food debris along make table (behind poster display) located near pizza oven. Dark buildup on flooring under cookline stainless steel tables. Discussed routine cleaning under and behind equipment. Buildup along warewashing machine and wall juncture. Buildup and splatter on warewashing machine (shelving; etc.). Food spillage in large shared storage room of basement. Spillage in cabinets under coffee maker. Discussed routine cleaning to prevent food/water sources for pests.

OBSERVATIONS & CORRECTIVE ACTIONS: Stuck on food debris on meat slicer being stored in small storage room in basement. Discussed ensuring item is cleaned before being placed in storage. Discussed stuck on food as a potential food source for pests. Instructed Person in Charge to have equipment washed, rinsed and sanitized. Advised storing in a clean location and/or wrapping.

OBSERVATIONS & CORRECTIVE ACTIONS: -Mice feces under 3-compartment sink -Mice feces under back storage shelf -Mice feces along corners of basement dry storage room -Mice feces in office -Mice feces in large shared storage room Discussed routine pest control services. Establishment should increase frequency and integrate different methods when evidence of pests is observed and especially after seeing any live activity. Person in Charge stated they observed a live mouse in the basement office recently. Person in Charge provided Pest Control invoice dates from10-13-14; 11-13-14; and 12-11-14. The actual invoice was not available as it was sent to the corporate office. Discussed reviewing pest control invoices to see what pests were treated for and other additional information important for sanitation. Inspector had staff begin to clean evidence of pests.
