OBSERVATIONS & CORRECTIVE ACTIONS: Observed some grease build up on the wall behind the fryer in the back prep area. Observed a water damaged ceiling tile in the back prep area. Observed some large holes in the wall in the back prep area. Observed excessive grease build up on the large pipes underneath the shelf with the fryer. Observed excessive debris and grease build up on the floors throughout the back prep area. Observed some small pieces of missing floor tiles underneath the shelf that holds the fryer. Observed some cracked floor tiles in the back prep area. Observed some missing floor tiles underneath the stove and flat grill. Observed some build up on the floor underneath the flat grill.

OBSERVATIONS & CORRECTIVE ACTIONS: Observed some sticky build up on the shelves above the four compartment sink. Observed some grease and food build up on the shelf with the fryer and also in the crease along the wall of that shelf. Observed some build up on the small blue storage shelf in the back prep area. Observed build up and spillage on the shelves and surfaces underneath the front prep counter below the plastic shield across from the Flat Grill. Observed some build up on the plastic shield above the front prep counter by the cash register.
