OBSERVATIONS & CORRECTIVE ACTIONS: Observed numerous mice droppings on the floor to the right of the beer storage racks by the Bakery Walk-in Freezer. Observed some mice droppings on the floor and in the corner underneath the stairs to the right of the mop sink. Observed mice droppings on the floor inside of the Dairy Walk-in Cooler behind some pallets. Observed some mice droppings on the ledge behind the milk crates inside of the Dairy Walk-in Cooler. Observed some mice droppings on the floor behind the orange steels in the Dairy Walk-in Cooler. Observed numerous mice droppings on the concrete ledge at the bottom of the pipe close to the wall across from the mop sink. Observed mice droppings on the floor underneath the storage rack by the store room to the right of the Produce Walk-in Cooler. Observed numerous flies throughout the front and back of the establishment.

OBSERVATIONS & CORRECTIVE ACTIONS: Observed numerous foods in the Lunchmeat Grab Cooler that were being held out of temperature. Meat Manager removed all of the foods counted them and discarded them. Foods are listed in the temperature section and notated with a double *. Observed numerous foods in the Produce Grab Cooler that were being held out of temperature. OBSERVATIONS & CORRECTIVE ACTIONS: Observed excessive debris on the floor underneath the beer storage racks in front of and to the right of the Bakery Walk-in Freezer. Observed some missing and broken floor tiles in the employee break room. Observed dried spillage and build up on the walls throughout the entire back storage areas. Observed mold build up on one of the windows on the door of the small Meat Walk-in Cooler with the swinging doors. Observed ice build up on the floor in the Bakery Walk-in Freezer. Observed debris on the floors underneath the pallets in the Dairy Walk-in Cooler. Observed debris on the floor underneath the orange steels in the Dairy Walk-in Cooler. Observed debris on the floor around the mop sink. Observed some the low wall by the cage room by the stairs is in disrepair. Observed excessive debris on the floor around the large electrical boxes and storage rack across from the receiving bay doors. Observed some debris underneath the storage rack to the right of the Produce Walk-in Cooler. Observed some rusted walls in the Produce Walk-in Cooler. Observed some rusted walls in the Dairy Walk-in Cooler. Observed some holes in the wall in the Produce Prep room. Observed some build up on the wall above the Meat Department three compartment sink. Observed some dust build up on the fan guards on the condenser above the Meat Department three compartment sink. Observed that some of the grout in between the wall tiles above the Meat department three compartment sink is missing. Observed different sections of the Meat Department prep room are peeling. Observed that the paint on the bottom of the condenser above the Meat Department three compartment sink is chipping. Observed that the latch on the door of the Large Walk-in Freezer is being held in place with a piece of blue wood. Observed that the door seal is in disrepair on the Bakery Walk-in Freezer. Observed a gap at the bottom of the Dairy Walk-in Cooler door. Observed several rusted racks in the Produce Walk-in Cooler. Observed that the handles of the restroom stalls in the Men's restroom are missing and tissue has been inserted in them. Observed some rusted racks in the Small Meat Walk-in Cooler.
