Observed 11 dead adult and juvenile roaches on top of the grease trap lid and floor next to grease trap, below the three-compartment sink. Observed 1 live juvenile roach on the dish machine. Observed 2 live adult roaches on the hot water heater, between the dish machine and the three-compartment sink. Observed 25+ baby, juvenile, and adult roaches scattering from inside a box of fortune cookies stored below the hot table. Observed 2 dead adult roaches on the floor behind containers of corn starch and rice. Observed 1 dead adult roach on the floor to the right of the fryers. Observed 2 dead juvenile roaches on the floor near the entry doors of the kitchen from the server station. Observed 30+ live baby, juvenile, and adult roaches on the shelving below the hot table that scattered when the cardboard and paper lining the shelf was moved. Observed the wall below the hot table to have a buildup of roach feces. **Pest control services received on 7/29/16 and 8/11/16 noted NO pest activity.
