OBSERVATIONS & CORRECTIVE ACTIONS: Observed the quat sanitizer bucket at the equipment line below 200 ppm. Person in charge remade quat sanitizer to a concentration between 200-400 ppm at time of inspection.

OBSERVATIONS & CORRECTIVE ACTIONS: Observed build up on the wall below the 3 compartment sink. Observed dust build up on the ceiling vents and surrounding ceiling throughout the kitchen. Observed debris on the floor under the fryers.

OBSERVATIONS & CORRECTIVE ACTIONS: Observed a single-service cup stored in a sauce in the left make table cooler being reused as a scoop. Person in charge discarded single service cup at time of inspection.

OBSERVATIONS & CORRECTIVE ACTIONS: Observed a container of grilled chicken and a container of cooked burgers being held on the grill top below proper hot holding temperature (see temperature log). Employee stated burgers and chicken were cooked about 45 minutes, and reheated both to be properly hot held at time of inspection.
