OBSERVATIONS & CORRECTIVE ACTIONS: Observed a hole in the wall by the pipes in dry storage. Observed splatter on the wall in dry storage. Observed part of the wall crumbling at the tilt cooker. Observed stained ceiling tiles in prep area. Observed grease on the floor by the cooking line.

OBSERVATIONS & CORRECTIVE ACTIONS: Observed vegan soup in the kitchen walk in cooler that was tightly wrapped before being allowed to cool to 41F (see temp log). Employee placed soup in an ice bath to allow to cool properly as it was within proper time constraints.

Employee placed items in the reach in cooler to return to proper cold holding temperature as it was within the four hour safety window. Observed condiments in an ice bath at the sandwich station that had an internal temperature above 41F (see temp log). Employee placed condiments back in the reach in cooler since it was within the four hour safety window.
